Conveniently located in the beautiful Plaza building, you’ll find us on the second floor in the same building as St. Joseph Health Family Medicine and other medical and professional offices. The Plaza at Lakewood is at the end of Lakewood Drive behind CVS/Safeway and just before Lakewood Hills Gated Community.

If heading north on US 101 take exit 498 toward Central/Windsor. Continue straight onto Lakewood Dr.  If heading south on US 101 take exit 498 toward Windsor, turn left onto Old Redwood Hwy. Turn left onto Lakewood Dr. Continue past Raley’s which will be on your right and make a left into the parking lot just before the Lakewood Hills gated community. We are on the second floor. Turn left if coming up the stairs, turn right when coming out of the elevator.
